Required Documents for Medical Visa Application

For the Patient:

  • Duly filled and signed Indian Visa Application Form. Apply Online
  • Original Sudanese passport (valid for at least 6 months with two blank pages)
  • Passport bio page photocopy
  • Two recent passport-sized photographs (2×2 inches, white background)
  • Yellow Fever and Oral Polio Vaccination certificates
  • Latest medical records and test results (MRI, CT scan, blood tests, etc.)
  • Referral letter from a registered medical institution in Sudan
  • Invitation letter from the Indian hospital, including the doctor’s name, treatment plan, and patient details
  • A recent bank statement or income certificate to demonstrate financial ability 
  • Copy of confirmed round-trip flight tickets

For Medical Attendants:

  • Copy of a valid passport
  • Proof of relationship to the patient (e.g., birth certificate, marriage certificate)
  • Proof of residence in Sudan (e.g., national ID, utility bill)

For Minors:

  • Birth certificate
  • Copies of both parents’ passports
  • Notarized visa application signed by both parents

Step 1: Online Application

Visit the Indian Visa Online portal: https://indianvisaonline.gov.in. Choose “Regular/Paper Visa Application.” 

Fill out the form with:

  • Page 1: Personal, passport, and family details
  • Page 2: Visa type (Medical), medical condition, hospital information in Sudan, and India
  • Page 3: Current address, contact details, and references in Sudan and India

Upload a recent photograph (or attach it later). Print the form, then sign under the photo and at the bottom of the last page.
